Prescribing. Prescriptions are issued by a GPhC-registered pharmacist independent prescriber, registered in Great Britain with the General Pharmaceutical Council (GPhC): Hari Jadeja, GPhC registration number [to be added]. You can check the registration of any pharmacy professional at pharmacyregulation.org.
Dispensing. Medicines are dispensed and supplied by our registered pharmacy partner: Smartway Pharma, 10 Lyon Road, London SW19 2RL, GPhC pharmacy registration number 9010774.

We reserve the right to decline to treat any patient at the sole discretion of the prescriber, without being required to provide a reason.
Hormox provides private healthcare consultations, followed by the prescription and dispensing of clinically appropriate treatments. Our service is not a substitute for your GP or for in-person care. We recommend that you remain registered with a GP, and we may ask for your GP's details so that we can prescribe safely.
Our service is not a substitute for emergency care. If you are experiencing a medical emergency, call 999 or attend your nearest Accident & Emergency department immediately. For urgent but non-emergency advice, call NHS 111.
All prescriptions are issued by a GPhC-registered pharmacist independent prescriber following a full clinical assessment. A prescription is not guaranteed following a consultation; the prescribing decision rests entirely at the discretion of the treating pharmacist, who may decline to prescribe where it would not be clinically appropriate or safe to do so.
We do not prescribe Schedule 2 or Schedule 3 controlled drugs. Some treatments we provide (for example, testosterone used in testosterone replacement therapy) are controlled drugs in other schedules under the Misuse of Drugs Act 1971 and associated regulations; these are prescribed only where lawful and clinically appropriate. Sharing with your GP. To prescribe safely we may ask for your GP's details and, with your consent (or where clinically necessary to protect your vital interests), share relevant information with your GP. We may decline to prescribe if we are unable to do so safely.
Side effects. Tell us immediately about any adverse effects, and report suspected side effects of any medicine through the MHRA Yellow Card scheme at yellowcard.mhra.gov.uk.
